Israel Health Declaration Form Tips and Tricks

*Please note this form is ever-changing, so we apologize in advance if any of these tips do not match your health form.*

Step 1: Start the form and then select arrival by air.

Step 2: Fill out your personal information. Whatever email you place here will receive the final submitted form.

Tip:
Make sure to change the response to, “Are you a citizen, permanent or temporary resident of Israel?” from Yes to No.

Tip:
The dates on this form are Day / Month / Year

Tip:
Start typing, “United States of America” in the country code box. Select the response and a 1 will populate.

Step 3: Fill out your arrival details.

Tip:
When choosing your airline, you can type or scroll to select. Please note you must put the flight details in for the flight that is arriving to Israel.

>Tip:
The date of departure and arrival are Day / Month / Year.

Tip:
The arrival time is in military time.

Tip:
Most travelers have not gone anywhere other than the US before this tour, but if you have, please fill out all the places you have been in the 14 days prior to this trip.

Step 4: Complete the Health Portion.

Tip:
Choose Vaccinated/Recovered abroad.

Tip:
Add all vaccine doses. Make sure to click to add more as you fill out your vaccination information.

Tip:
Click the arrow in the black circle to upload a photo of your vaccine card. The form prefers a PDF file. You will need to click "Continue Filling Out the Form" after clicking the black circle with the white arrow.

Tip:
Click that you are aware of the isolation requirements (after reading them). Choose the proper test you took or are taking before travel, and finally, confirm that all the details are correct.

Step 5: Review the information you filled out.

Tip:
You do not have a group code. Please do not place anything in this field and continue on to the next section.

Step 6: Fill out your initial isolation details.

Tip:
This isolation address is the first ISRAEL hotel you will stay in. If you go to Bethlehem first, fill out the first hotel where you are staying in Israel.

Tip:
This address can be found in your final electronic documents. If the street is not an option on the drop-down, begin typing it and click “The Street Is Not On The List”. After clicking that option, click the street box again and manually type in the street.

Tip:
Make sure to fill out the house (in your case, a hotel) number and then click “I read and understood the isolation requirements”.

Step 7: You are approved!

Tip:
Scroll down to the bottom of the page, download, and print the status and Green Pass pages. These are emailed to you as well.

If for some reason you are denied, don’t worry, please try again and follow these tips closely and redo the form.
